Administrative and Office Positions

Client Services Specialist:  Responsible for information management relative to the receipt, distribution and update of client records and documentation both medical and financial. Requirements: 2-4 years of administrative experience in social or aging services or healthcare; excellent detail orientation, organization, planning and computer skills.

Client Services Project Manager (I and II):  Responsible for ensuring that client care needs are achieved through communication with clients and internal and external care team partners. Projects include: ongoing care plan updates, communication with care team partners, service and appointment scheduling and related follow-ups to ensure above average client care. Requirements: 4-6 years of administrative and direct client service experience in social or aging services or healthcare; project planning and management, detail orientation, initiative, and creative problem solving skills.

Client Services Supervisor: Responsible for managing office client service staff in the roles of project manager and specialist and working with community (field) staff relative. This position provides oversight to ensure quality control related to client care and documentation, project completion, external client and care team partner communication and reporting. This position requires 5-7 years of administrative and direct client service experience in social or aging services in addition to the ability to manage unexpected, time sensitive, and client situations related to fiduciary and legal oversight. Additional requirements are: exceptional customer service skills, problem solving, attention to detail and supervisory skills.

Client Services Director: Responsible for managing all administrative and office staff and working with community (field) staff relative to ensuring that client care exceeds industry standards.  This position is responsible for the management, development, oversight and initiation in the areas of operations, fiduciary oversight, program development, and client care. This position requires 7-10 years of management, supervisory, organizational management, and customer service skills in an aging, in social services or in a healthcare environment.

All positions with The Care Navigator bear the duty of fiduciary responsibility to our clients. This means that all employees have a legal and ethical responsibility to act in the best interest of our clients being both effective and efficient with the time we devote to projects.  Our clients entrust us with responsibilities related to both healthcare and financial matters. All positions with the company are time accountable meaning that time is documented and is billable to our clients.

Community (Field) Positions

Note that these are combination positions (both requiring administrative skills and direct field client service experience)

Client Care Associate (I and II): Responsible for providing care planning, coordination and care oversight for clients in the areas of health care, home care, medical and related services, supporting all types of daily living activities, and working with care team partners to ensure that client care exceeds industry standards. This position requires 3-6 years of both administrative experience and direct field client experience working in the community in the area of aging services specializing in client care management for the aged, those diagnosed with dementia, physical disabilities, mental illness, the mentally disabled, and other health diagnoses. Other requirements: a high level of customer service, organization, planning, detail orientation, documentation (progress notes), and computer skills.

Client Care Manager (I and II): Responsible for providing care planning, coordination and care oversight for clients in the areas of health care, home care, medical and related services, supporting all types of daily living activities, and working with care team partners to ensure that client care exceeds industry standards. This position supports training for individuals in the Client Care Associate position. This position requires 5-7 years of both administrative experience and direct field client experience working in the community in the area of aging services specializing in client care management for the aged, those diagnosed with dementia, physical disabilities, mental illness, the mentally disabled, and other health diagnoses. Supervisory experience is helpful in addition to a high level of customer service, organization, planning, detail orientation, documentation and computer skills.

Client Care Team Leader: Responsible for providing care planning, coordination, care oversight, and supervisory visits for clients in the areas of health care, home care, medical and related services, supporting all types of daily living activities, and working with care team partners to ensure that client care exceeds industry standards. This position identifies and supports care planning, training, and ongoing development of team members in the Client Care Associate and Client Care Manager positions. This position requires 7-10 years of administrative, customer service, direct field, supervisory, and client care experience working in the community in the area of aging services specializing in client care management for the aged, those diagnosed with dementia, physical disabilities, mental illness, the mentally disabled, and other health diagnoses.

Client Care Director: Responsible for management, supervision, and oversight of field and fiduciary responsibilities. This position also includes: client intake meetings, initiating care planning, providing direct client service, contact with external care team partners, and coordination with the administrative office team to deliver exceptional care. This position requires 10-15 years in a care management and administrative or operational position that includes the skills of customer service,  care partner relations, supervision, operations, and direct field care management experience working in the community in the area of aging services specializing in client care management for the aged, those diagnosed with dementia, physical disabilities, mental illness, the mentally disabled, and other health diagnoses. Experience in fiduciary or legal services is beneficial.
