Colorado Care Management for the Disabled in Denver and the Front Range

The staff of The Care Navigator provides care management services in Denver and the Colorado Front Range for the young and older disabled.  The term disabled is a broad category and benefits from definition. The Care Navigator care management for the disabled specializes in the category of developmental disabilities, learning disabilities and adults who were born with a disability at birth.  The Care Navigator does not specialize in individuals needing quadriplegic care or those with specialized medical care i.e. ventilators etc.

The Care Navigator staff serve in the role of a care manager for young and older disabled adults and possess the expertise of working with persons ages 25 and older. Many of our clients were born with learning disabilities or acquired disabilities as the result of an accident. These types of disabilities require services specializing in the disability.

We have clients living in group home situations or in an apartment or similar arrangement with supports. Some of our disabled clients function well day to day and need only an occasional check in. Other disabled clients receive care management services that connect them with community resources. Vocational rehab, brain injury programs, learning and educational programs are some of the ancillary providers with whom we work.

Each situation and need is different. Some clients focus on vocational efforts of wanting to obtain a job. Other clients focus on learning for example, learning to cook or manage an allowance. We work with ancillary providers who are able to initiate assessments and make a recommendation for a plan of care for our young and older disabled clients.
